Inflammation and Rare DiseaseNarrow moat
Amgen (AMGN) — moat facet
Amgen rebuilt its inflammation line with $41 billion of purchases as ENBREL fell by more than half, and the band now barely grows on a full-year basis.
Inflammation and rare disease is the line Amgen bought. It sold $11,045 million in 20251, about 30.1% of total revenues2. Inside are ENBREL, $2,226 million; Otezla, $2,265 million; TEZSPIRE, $1,478 million; and the Horizon and ChemoCentryx drugs: TEPEZZA $1,903 million, KRYSTEXXA $1,340 million, UPLIZNA $655 million, TAVNEOS $459 million and the ultra-rare products $719 million3.
Its history is replacement by purchase. In 2019 the band was $5,404 million, almost all ENBREL4. ENBREL then shrank every year, from $5,226 million in 2019 to $2,226 million in 202556. Otezla, bought in November 2019 for $13.4 billion7, lifted the band to $7,191 million in 20208; Horizon, bought in October 2023 for about $27.8 billion9, lifted it to $10,870 million in 202410, up about 44%11.
How it is paid varies by drug. ENBREL and Otezla are sold into crowded classes through benefit managers and are now priced by Medicare, from 2026 and 202712. The rare-disease drugs sell to specialists with little competition, and there prices rise: KRYSTEXXA's net selling price was up 23% in the second quarter of 202613. Amgen reports no profit by line, and the acquired drugs carry heavy amortisation14.
Growth has slowed as the purchases were absorbed: about 1.6% in 202515. The second quarter of 2026 was better, $3,167 million against $2,887 million, about 9.7%16, as the rare-disease drugs and TEZSPIRE grew faster than ENBREL and Otezla fell.
The outlook is a race between the old and the bought. ENBREL and Otezla, $4,491 million in 202517, face government prices and copies; the rest of the band, $6,554 million18, is growing.
The band holds both Amgen's strongest pricing and its weakest. TEPEZZA's second-quarter growth came from 6% higher net selling price and 6% volume growth, and KRYSTEXXA's from 23% higher net selling price19, while Otezla's net price fell 9% and ENBREL's 22%20. The rare-disease drugs are where the purchases are paying.
The line passes its test if the growing part keeps outrunning the declining part. If the band's growth falls back toward zero in 2027, when Otezla's Medicare price arrives, Amgen will have bought a portfolio that only replaces what it loses.
Up about 10% in Q2 2026 after 1.6% in 2025.
The bought portfolio against ENBREL and Otezla; growth near zero in 2027 would mean it only replaces losses.
