Repatha: From $661 Million to $3.0 BillionNarrow moat

Amgen (AMGN) — moat facet

Repatha grew 4.6 times between 2019 and 2025 by selling more at lower prices, and it is the largest drug Amgen built itself in a decade.

Repatha is Amgen's best evidence that it can still grow a drug of its own. It lowers LDL cholesterol by blocking a protein called PCSK9. It sold $661 million in 2019 and $3,016 million in 202512, about 4.6 times3, and $953 million in the second quarter of 2026, up 37%4.

A slow start, then a blockbuster.

Repatha was launched in 20155, so it took about a decade to pass $3 billion. Its growth is a volume story: Repatha grew about 36% in 20256, the second-quarter 2026 increase was driven by volume growth7, and the 10-K expects its net selling price to decline by approximately mid-single digits in 20268. In August 2025 the FDA broadened its approved use to adults at increased risk of major cardiovascular events from uncontrolled LDL cholesterol, removing the requirement of a prior diagnosis of cardiovascular disease9.

The sales are spread across the world: $1,663 million in the United States and $1,353 million elsewhere in 202510. Amgen also sells Repatha directly to patients through AmgenNow11.

The newest support is clinical. In the VESALIUS-CV study, patients who had had a coronary stent or angioplasty saw a 30% relative reduction in the risk of a composite of coronary heart disease death, heart attack or ischemic stroke12. Evidence of that kind moves prescribing among cardiologists more than any sales call.

Repatha is also one of the few large Amgen drugs sold almost evenly at home and abroad: $510 million in the United States and $443 million elsewhere in the second quarter of 202613. That spread makes it less exposed than ENBREL or the Horizon drugs to any single government's pricing rules.

Repatha is protected by antibody patents to August 2029 and method patents to May 2032 in the United States14. It is the most important product Amgen built rather than bought. Its quarterly growth rate, 37% in the second quarter of 202615, is the figure that matters; a fall below 15% before 2029 would mean it matured early and faces its patent expiry from a lower peak.

Moat trajectory: Widening

Up 37% in Q2 2026; outcomes data supportive.
