Medicare sets ENBREL's price from 2026 and Otezla's from 2027, and Amgen has already written Otezla down by $1.2 billion because of it.
The Inflation Reduction Act allows Medicare to set prices for older drugs, and Amgen has two on the list. CMS has selected ENBREL and Otezla for Medicare price setting beginning in 2026 and 2027, respectively1. Amgen also says several of its products have been subject to the law's inflation rebates2, and that the impact is expected to increase through the end of the decade3.
Amgen has not disclosed the negotiated prices or their dollar cost. What it has shown is the result. ENBREL's net selling price fell 22% in the second quarter of 2026, reflecting Part D price setting effective January 1, 20264, and Otezla's intangible was impaired by $1.2 billion once it was selected5. The general mechanics are described on AbbVie's Prices Set in Washington page and Merck's Medicare page; for Amgen the relevant question is which drug comes next.
The candidates are its largest products that have been on the market for years. Prolia and XGEVA are already losing to biosimilars. Repatha, launched in 2015 and now selling $3,016 million a year6, is the obvious next exposure.
The two selected drugs sold $4,491 million in 20257. A third selection, particularly Repatha, would turn this from a problem of ageing drugs into a problem of growth drugs, and that is the event to watch for in each January's list.
