Merck & Co. (MRK) — moat facet

Merck's other human medicines sell $21 billion a year, and each launch is racing an exclusivity loss elsewhere in the portfolio.

The rest of the Pharmaceutical segment is a portfolio in transition. After Keytruda and Gardasil, Merck's human medicines and vaccines sold $21,228 million in 20251, about 32.7% of total sales2. In the second quarter of 2026 this portion sold about $5,225 million, up about 5.2%3.

A portfolio of launches and fading drugs.

What is inside is dozens of products at different stages of life. Growing: Winrevair $1,443 million, Welireg $716 million, Prevymis $978 million, Capvaxive $759 million and Vaxneuvance $825 million in 20254. Steady: Bridion $1,841 million, the childhood vaccines $2,451 million, RotaTeq $673 million5. Fading: Januvia $1,604 million and Janumet $940 million, Lagevrio $380 million, Pneumovax 23 $166 million6. Alliance revenue from Lynparza, $1,450 million, and Lenvima, $1,053 million, and royalties on Reblozyl, $525 million, are also here7.

The history shows the churn. This portion was $19,895 million in 2021, rose to $24,171 million in 2022 on the COVID antiviral Lagevrio, which sold $5,684 million that year8, and fell to $19,686 million in 20239. Simponi and Remicade, which sold $710 million and $187 million in 202310, went back to Johnson & Johnson when their marketing rights in former Merck territories returned on 1 October 202411.

How it is paid varies by product, but the channels are the same wholesalers, hospitals, government agencies and benefit managers as for Keytruda12; alliance products book only Merck's share of profits13.

Profitability is not disclosed by product; the segment margin was about 78.7% in 202514. Several products are not wholly Merck's: it pays a 22% royalty on Winrevair to Bristol Myers Squibb15.

Growth is a race between launches and exclusivity losses. Januvia and Janumet lost American exclusivity in 2026 and fell 31% in the second quarter1617; Bridion lost it in July 202618; Lynparza and Bravecto follow in 202719.

Two products in this line illustrate its churn. Lagevrio, the COVID antiviral, sold $952 million in 2021, $5,684 million in 2022 and $380 million in 20252021, so the line's 2022 peak was a pandemic rather than a franchise. Welireg, a kidney cancer drug, went from $13 million in 2021 to $716 million in 20252223, and grew 67% to $271 million in the second quarter of 202624: the kind of product that must multiply for this line to matter after 2028.

The alliances in this line cut both ways. Merck books only its share of profits on Lynparza with AstraZeneca and Lenvima with Eisai25, which makes their reported figures smaller and more profitable than an owned product of the same size. Lenvima has been selected for a Medicare price effective in 202826, and Lynparza's American protection ends in 202727, so two of the line's larger pieces face their own dates before Keytruda's.

The outlook depends on Winrevair and the acquisitions. The quarterly sales of this portion, about $5.2 billion in the second quarter of 202628, are the figure to hold it to; growth that turns negative in 2027 as Bridion and the diabetes drugs fade would mean the launches are not yet large enough to carry the portfolio, let alone Keytruda.
