Gardasil lost 39% of its sales in a year because one Chinese distributor stopped buying.
Gardasil was Merck's second pillar until 2025. The HPV vaccine family sold $5,673 million in 2021, $6,897 million in 2022, $8,886 million in 2023 and $8,583 million in 2024, then $5,233 million in 202512, a fall of 39%3.
Almost all of the fall was outside the United States. International Gardasil sales went from $6,803 million in 2023 and $6,158 million in 2024 to $2,592 million in 20254, down about 57.9% in one year5. American sales rose to $2,641 million6.
The cause was China. Because of above-normal inventory at its commercialisation partner, Merck paused shipments to China beginning in February 20257, and its annual report said it "has not resumed shipments to date"8. Merck's whole China business fell from $6,802 million of sales in 2023 to $1,939 million in 20259, a story told on the root threat page.
The vaccine itself did not change. What collapsed was a single channel in a single country that had become roughly half of Gardasil's sales. A moat built on manufacturing a complex vaccine does not protect against a distributor's warehouse filling up.
Merck's own forecast is cautious. It expects that sales of Gardasil in China "will not materially increase in 2026"10, and when it began limited shipments under a revised contract in the second quarter of 2026 it said the associated revenue was expected to be immaterial this year11.
The franchise is now mostly American and European again. The number to follow is international Gardasil sales, $2,592 million in 202512: a return toward $4 billion would mean China is buying again; another year near this level means the China business is not coming back in its old form.
