Merck's pneumococcal vaccines sell about a quarter of Pfizer's Prevnar, and they are winning adults while losing children.

In pneumococcal vaccines Merck is the challenger. Pfizer's Prevnar family sold $6,494 million in 2025, $4,151 million of it in the United States1. Merck's two conjugate vaccines, Capvaxive and Vaxneuvance, sold $759 million and $825 million2, together $1,584 million3, about a quarter of Prevnar's sales4.

The incumbent is four times larger.

The two fronts are going different ways. Capvaxive, Merck's adult vaccine, sold $184 million in the second quarter of 2026, up 42%5. Vaxneuvance, used mainly in children, fell 35% to $148 million, which Merck attributes partly to lower demand "due to competitive pressure"6.

This is the vaccine business as a contest between two firms with the manufacturing scale to compete. Neither can easily enter the other's position quickly; each new generation of vaccine covers more strains and takes years to develop and license.

Merck's older product, Pneumovax 23, fell to $166 million in 20257, so its pneumococcal business depends on the newer vaccines winning share from an incumbent.

The children's vaccine was a real gain before it turned. Vaxneuvance sold $3 million in 2021, $170 million in 2022, $665 million in 2023, $808 million in 2024 and $825 million in 202589, so the 2026 decline follows four years of share taken from the incumbent.

Merck is gaining in adults and losing in children. Its combined pneumococcal share against Prevnar, about 24% in 202510, is the figure to watch; a rise toward a third would mean Capvaxive is changing the adult market, while a fall would mean Vaxneuvance's losses outweigh it.
