Keytruda is 49% of Merck's sales, and every year it outgrows the rest of the company the eventual loss gets larger.

Merck's largest risk is the success of its largest product. Keytruda represented 49% of total sales in 20251, and the company names it first among the key products on which its profits and cash flows "depends largely"2.

The concentration rose steadily. Keytruda was about 35.3% of sales in 2021 and about 41.6% in 20233. Because the rest of the Pharmaceutical segment lost Januvia sales and COVID-era Lagevrio, which sold $5,684 million in 2022 and $380 million in 202545, the franchise's share kept climbing even in years its own growth slowed.

The company's own general warning applies with full force: when products lose exclusivity, Merck experiences a significant and rapid loss of sales6. At 49% of sales, a Humira-like fall would take a large share of the company's revenue within a few years.

Merck's own risk factors read like a description of this page. The company states that key products generate a significant amount of its profits and cash flows, and that any events adversely affecting the markets for its leading products could have a material adverse effect7.
