Merck & Co. (MRK) — the future bets

Merck's post-Keytruda bets have strong trial results and a July 2026 approval, and together the launched successors sell a seventh of Keytruda.

Merck's future bets are its answer to one question: what replaces Keytruda's $31.7 billion12 after 2028? The candidates on this page are four very different kinds of wager.

Growing, and still small beside Keytruda.

The first is sac-TMT, an anti-TROP2 antibody-drug conjugate developed in collaboration with Kelun-Biotech, with 17 ongoing global Phase 3 trials3. The second is intismeran autogene, a personalised cancer vaccine developed with Moderna and given with Keytruda, which met both primary endpoints in a Phase 3 melanoma trial of 1,137 patients in August 20264. The third is Lipfendra, approved by the FDA in July 2026 as the first once-daily oral PCSK9 inhibitor5. The fourth is internal: a programme to save $3.0 billion a year by the end of 2027 and reinvest all of it in growth areas6.

They are bets because none yet has meaningful sales. The launched successors that do, Winrevair, Welireg, Ohtuvayre and Capvaxive, sold about $1,247 million together in the second quarter of 202678, against $8,366 million for the Keytruda franchise9.

The market has started to price some of them. Merck's shares rose about 12% on the intismeran result10, and were up about 85% over the year to September 202611.

Other bets are further back. Merck is developing an oral once-weekly HIV treatment with Gilead, islatravir with lenacapavir, and presented the first Phase 3 results in July 202612. It also expanded Keytruda's use with Padcev in muscle-invasive bladder cancer in July 202613. None of these has a sales line yet, which is why the launched products and the acquisitions carry most of the replacement plan today.

The bets are promising and unproven. Merck's non-GAAP operating expense guidance for 2026 rose to $42.0 to $42.7 billion from $36.0 to $36.8 billion, largely on the acquisition charges14; the figure to hold the portfolio to is the sales of the newer products, about $1.25 billion a quarter now, which needs to be several times larger by 2029 for the replacement to work.
