Johnson & Johnson (JNJ) — moat facet

J&J's device business earned about 27 cents before tax per dollar of sales in 2015 and about 12 cents in 2025.

MedTech is J&J's device business: a third of sales and about a sixth of segment profit. It sold $33,792 million in 2025 and earned $4,113 million before tax, a 12.2% margin1. Innovative Medicine earned 36.9%2.

The margin has more than halved in a decade.

What is inside it. The segment develops products used in cardiovascular, orthopaedic, surgery and vision fields3. In 2025 surgery sold $10,137 million, orthopaedics $9,258 million, cardiovascular $8,928 million and vision $5,468 million4. Cardiovascular includes electrophysiology, $5,634 million; Abiomed heart pumps, $1,751 million; and Shockwave, $1,146 million5. Vision is mainly contact lenses, $3,910 million, and surgical lenses, $1,558 million6.

History. Until 2022 the segment was reported as Medical Devices. It sold $25,137 million in 20157, $26,994 million in 20188, $22,959 million in 20209, and $27,060 million in 202110. The portfolio has been rebuilt by trading: Abbott Medical Optics was acquired in February 201711, LifeScan was divested in the fourth quarter of 201812, Abiomed was bought for $17.1 billion net of cash in December 202213, and Shockwave for $12.6 billion in 202414. Orthopaedics is now being separated15. From 2015 to 2025 sales grew at about 3.0% a year16.

How it is paid. Hospitals, health systems and, abroad, government purchasers buy devices under negotiated contracts and tenders. The segment accrued $6,446 million of rebates in 2025, about 19% of sales1718, and faced volume-based procurement in China across advanced surgery and orthopaedics19.

Profitability. This is the weak point. Medical Devices earned $6,826 million before tax on $25,137 million of sales in 2015, about 27%2021. On the restated basis MedTech earned 15.6% in 2021 and 16.2% in 20222223, then 15.4% in 2023, 11.7% in 2024 and 12.2% in 202524. Acquisition costs, amortisation, litigation such as the $0.9 billion mostly for the Auris shareholder case, and restructuring all weigh on it25. The segment also used $86,482 million of identifiable assets in 2025, more than Innovative Medicine's $78,057 million26. In the second quarter of 2026 the margin was about 13.2%2728.

Growth. Restated sales rose 1.4% in 2022, 10.8% in 2023 including Abiomed, 4.8% in 2024 and 6.1% in 2025293031. In the first half of 2026 the segment grew 6.0% and in the second quarter 4.5%32. The weak spots are surgery, up 3.0% in 2025, and orthopaedics, up 1.1%33.

Outlook. J&J's plan is to concentrate on cardiovascular, surgery and vision after the orthopaedics separation, a shift it describes as toward higher-growth and higher-margin markets34. The Ottava surgical robot, authorised in July 202635, is the main new product. Tariffs and currency pushed up the segment's cost of products sold in 202536.

The cost structure is the mirror of the medicines business. In 2025 MedTech's cost of products sold was $14,549 million, selling, marketing and administrative expenses $11,354 million and research and development only $2,838 million37, 8.4% of segment sales against 19.6% for Innovative Medicine38. Devices spend less on research and more on making and selling, which is the profile of a business that competes on execution rather than invention.

The first half of 2026 showed the pressure continuing. MedTech sold $17,562 million, up 6.0%, but earned $2,416 million before tax against $2,625 million a year earlier3940, a margin of about 13.8% against 15.9%41. Selling and administrative costs rose 8.3% and research 12.7%4243, both faster than sales.

The segment's balance between home and abroad is closer than the medicines business: United States MedTech sales were $17,408 million in 202544, about 52% of the segment45.

The segment's research budget is also shrinking as a share of sales. MedTech spent $2,838 million on research and development in 2025, 8.4% of segment sales, against $3,703 million, 11.6%, in 202446, when acquired research for V-Wave inflated the figure. Devices are cheaper to develop than medicines, which is part of why they earn less.

What remains is a narrow moat that earns less than half of what it did a decade ago on each dollar of sales. The figure that would change the verdict is the MedTech margin in the first full year without orthopaedics: above 18% would show the reshaping restored the business's economics, while a margin still near 12% would mean the problem lies in the devices J&J is keeping.
