J&J's medicines business is 64% of sales and about 84% of segment profit, and its margin rose to 36.9% in the year its second-largest drug collapsed.
Innovative Medicine is J&J's medicines business, and it is where nearly all the profit is made. It sold $60,401 million in 2025, 64% of the company, and earned $22,266 million before tax, a 36.9% margin12. MedTech earned $4,113 million3, so the medicines business produced about 84% of segment income4.
What is inside it. The filing lists six therapeutic areas: oncology, immunology, neuroscience, pulmonary hypertension, infectious diseases, and cardiovascular and metabolism5. In 2025 oncology sold $25,380 million, immunology $15,728 million, neuroscience $7,837 million, pulmonary hypertension $4,437 million, infectious diseases $3,241 million and cardiovascular, metabolism and other $3,778 million6. The three largest products, Darzalex, Stelara and Tremfya, were about 27% of the whole company's revenue78. The products are distributed to retailers, wholesalers, distributors, hospitals and healthcare professionals for prescription use9.
History. Until 2022 the segment was reported as Pharmaceutical, one of three segments alongside Consumer Health and Medical Devices10. It sold $31,430 million in 201511, $42,198 million in 201912 and $52,080 million in 2021 on the basis reported that year13. After the Kenvue separation it was renamed Innovative Medicine; restated to continuing operations, it sold $51,680 million in 2021 and $52,563 million in 202214. From $31,430 million to $60,401 million is a compound rate of about 6.8% a year over ten years15, on bases that differ slightly.
How it is paid. J&J sells at a list price and then pays large rebates to payers. The segment accrued $56,819 million of rebates in 2025, about 94% of its net sales1617. The reported revenue is what remains after those deductions. Where J&J licenses a product it also pays royalties out: about $2.4 billion to Genmab on Darzalex in 202518.
Profitability. The segment margin was 33.3% in 2023, 33.2% in 2024 and 36.9% in 202519. In 2025 it spent $11,827 million on research and development, 19.6% of sales, down from $13,529 million, 23.8%, in 2024, when acquired research charges were larger20. The old Pharmaceutical segment earned between about 21% and 38% before tax from 2015 to 2021, with the 2019 low of $8,816 million on $42,198 million of sales including litigation charges2122. In the second quarter of 2026 the margin was about 38.1%2324.
Growth. Restated sales rose 1.7% in 2022, 4.2% in 2023, 4.0% in 2024 and 6.0% in 2025252627, the last despite Stelara costing about 10.4 points of operational growth28. In the first half of 2026 the segment grew 9.4% and in the second quarter 7.8%29. The slower years were 2022 to 2024; immunology was flat in 2024, at $17,828 million against $18,052 million, before Stelara's decline began30.
Outlook. Oncology is the engine: up 22.1% in 202531 and 17.3% in the second quarter of 202632. The four myeloma medicines alone sold $5,298 million in that quarter33. Against it stand the expiries: Stelara still falling, a Simponi biosimilar pursuing approval in the United States, generic competition for Opsumit expected in 2026, and Darzalex's United States patents expiring in 202934.
The segment's cost structure explains its margin. In 2025 its cost of products sold was $15,646 million, selling, marketing and administrative expenses $11,375 million and research and development $11,827 million35, against sales of $60,401 million. The cost of making the medicines is about a quarter of sales36; the rest of the expense is selling and research, which are the two activities the moat depends on.
The first half of 2026 shows the margin holding. Segment income before tax was $11,566 million on sales of $31,810 million, about 36.4%3738, against $10,762 million on $29,075 million a year earlier39. Research and development rose to $5,688 million from $5,417 million40, slower than sales, while selling, marketing and administrative expenses rose to $6,048 million from $5,050 million41, faster.
The geography of the segment is shifting toward the United States. United States medicines sales were $36,344 million in 2025 against $31,169 million in 202342, growth of about 16.6%43, while international sales grew more slowly. The segment is becoming more exposed to the market where prices are most contested.
This is a wide-moat business whose width depends on replacing its largest products every few years. Operational growth above 5% through 2027, with Stelara, Simponi and Opsumit all declining at once, would confirm the replacement engine; a year below 3% would show the expiries catching up with it.
Oncology growth is covering Stelara's loss; more expiries are due.
The medicines margin; a fall back toward 33% would mean replacements earn less than the products they replace.
