Johnson & Johnson (JNJ) — the future bets
J&J has promised double-digit growth by 2030 from a business that has compounded at 6.7% for five years.
J&J's most important future bet is a sentence. In April 2026 the company described itself as solidifying a path to double-digit growth by the end of the decade1, and announced an Enterprise Business Review for 8 December 20262.
The distance is large. J&J's five-year compound annual sales growth to 2025 was 6.7% and its ten-year rate 5.2%3. Its 2026 guidance calls for about 7.3% reported growth and 6.5% adjusted operational growth4. Double digits would mean roughly half again the pace of the last five years.
The sources would have to be the fastest-growing parts of the portfolio. Oncology grew 22.1% in 20255 and 17.3% in the second quarter of 20266. Cardiovascular grew 15.8% in 20257. Removing orthopaedics lifts the MedTech growth rate. Against that stand the patent losses already described: Stelara, then Simponi and Opsumit, then Darzalex in 2029.
The ambition also has to survive the costs of reaching it. The July 2026 deals reduced 2026 adjusted EPS guidance to $11.04, only about 2.3% above 2025 at the midpoint8. Growth in sales at the cost of flat earnings would not be the kind the market is paying for.
The quarterly numbers suggest the base is moving the right way. Adjusted operational sales growth was 5.3% in the first quarter of 2026 and 5.7% in the second910, and J&J raised its 2026 guidance for adjusted operational sales growth to 6.2%-6.8%11. But adjusted EPS fell 2.5% in the first quarter12, a reminder that sales growth and earnings growth are different targets.
The ambition is plausible for sales and unproven for earnings. Adjusted operational sales growth is the pace to beat, 6.5% at the 2026 guidance midpoint13; it would need to be above 8% in 2027 for double digits by 2030 to look like a trajectory rather than a target.
Guidance was raised twice in 2026 before the deals; the Enterprise Business Review is due in December.
The pace against the double-digit ambition; above 8% adjusted operational growth in 2027 would make it a trajectory.
