J&J's sales grew 10.6% in two years and its operating cash grew 7.6%.

Operating cash has not kept pace with the business. Between 2023 and 2025 sales rose from $85,159 million to $94,193 million, 10.6%, while operating cash flow rose from $22,791 million to $24,530 million, 7.6%12.

Cash lagged the top line.

Part of the difference is litigation cash. J&J paid the $0.8 billion Auris judgment in January 20263 and still owed approximately $3.7 billion related to talc and $0.9 billion related to opioid settlements at the end of June 20264. Part is working capital: receivables rose from $14,842 million to $17,178 million and inventories from $12,444 million to $14,191 million in 20255.

A slower-growing cash flow matters because the claims on it are growing. Dividends rose 5.2% over the same two years67.

Interest is a new drag. Interest expense rose to $971 million in 2025 from $755 million, and interest income fell to $1,056 million from $1,332 million8. For most of the past decade J&J earned more interest than it paid; that margin has narrowed to less than $100 million910.

Working capital absorbed some of the growth. Receivables rose by about $2.3 billion and inventories by about $1.7 billion in 20251112, together about $4.1 billion of cash tied up in the business as sales grew and acquisitions were integrated. Some of that should reverse; if it does not, the gap will persist.

The measure is the gap between sales growth and cash flow growth. Closing it over 2026 would mean the drag was working capital and litigation; a wider gap would mean the franchises are converting less of their sales to cash.
