MedTech is a third of J&J's sales and about a sixth of its segment profit, and it uses more assets than the medicines business.

MedTech is the half of J&J that most people picture and the half that earns the least. The segment sold $33,792 million in 2025, 36% of the company12, and earned $4,113 million of income before tax, a margin of 12.2%3. Innovative Medicine earned 36.9% on its sales4. MedTech produced about 16% of J&J's segment income5.

Cardiovascular did almost all the growing.

The moat in devices is real but different. It rests on surgeons and hospital staff trained on J&J's systems, on breadth across the operating room, and on a sales force that is present during procedures. Those advantages are sticky, but they do not command drug-like prices. MedTech accrued $6,446 million of rebates in 20256, about 19% of its sales7; the hospital customers bargain hard.

The four franchises have diverged. Cardiovascular grew from $6,350 million in 2023 to $8,928 million in 2025, helped by Abiomed and Shockwave8. Surgery, $10,137 million, and orthopaedics, $9,258 million, barely moved over the same two years9. Vision grew 6.3% to $5,468 million10.

J&J's response is to reshape the portfolio. The orthopaedics separation is meant to accelerate the shift toward higher-growth and higher-margin markets11, and after it J&J will keep a leadership position in six growth areas, of which three are in MedTech: cardiovascular, surgery and vision12.

The segment also carries costs medicines do not. MedTech's cost of products sold rose in 2025 on tariffs, unfavourable transactional currency and macroeconomic factors13, and it used $86,482 million of identifiable assets against Innovative Medicine's $78,057 million14.

The capital intensity shows in the spending. MedTech invested $2,501 million in property, plant and equipment in 2025 against $2,076 million for Innovative Medicine15, more capital for about a sixth of the pre-tax profit. Devices are made in factories that must be kept current, sold with instruments and consoles, and serviced; medicines, once developed, are cheap to make.

Some of the recent weakness is one-off. MedTech's 2025 results included litigation expense of $0.9 billion, primarily related to the Auris shareholder litigation, and a restructuring-related charge of $0.5 billion16; the 2024 results included about $1.0 billion related to the Shockwave acquisition17. Without those items the margin would be several points higher, but those items have recurred in different forms for three years.

The first half of 2026 made the point sharper: MedTech segment income before tax fell to $2,416 million from $2,625 million while Innovative Medicine's rose to $11,566 million from $10,762 million18.

MedTech is a narrow moat carrying a heavy balance sheet. Its margin after the orthopaedics separation will decide: if the remaining business does not earn clearly more than 12.2%, the separation will have shrunk MedTech without improving it.
