Johnson & Johnson lost 41% of its second-largest drug in 2025 and its medicines business still grew 6%.
A pharmaceutical franchise is a melting asset. The patent ends, copies arrive, and the sales fall away within a year or two. What separates a durable drug company from a lucky one is whether it can replace each product before the melt, and 2025 was a clean test of J&J's ability to do it.
The test was Stelara. It had been J&J's second-largest product, approximately 11.7% of revenue in 20241, and biosimilar competition arrived in 2025. The drug fell 41.3% to $6,078 million2, and the filing puts the damage at approximately 10.4% of worldwide Innovative Medicine operational sales3. The segment grew 6.0% anyway, to $60.4 billion4.
The replacements came from several directions at once. Oncology rose 22.1% to $25,380 million, led by Darzalex and Carvykti5. Tremfya, Stelara's intended successor, grew 40.5% to $5,155 million6. Neuroscience grew 10.1% after the purchase of Intra-Cellular7. Between 2023 and 2025 Darzalex, Tremfya, Carvykti, Erleada and Spravato together added about $10.2 billion of sales against Stelara's $4.8 billion loss8.
The machine has three parts. One is research: J&J spent $11,827 million on Innovative Medicine research and development in 2025, 19.6% of segment sales9. The second is purchase, covered on the next page. The third, less visible, is breadth: the filing says there was no individual project representing more than 5% of total annual research and development expense apart from one acquired asset in 202410. No single failure can sink the programme.
The limit of the machine is that each replacement is priced into a harder world. Price subtracted 3.1 points from J&J's growth in 2025 while volume added 8.411. J&J is replacing products successfully but selling more units at lower prices to do it.
The speed of the handover is the part rivals find hardest to copy. Stelara's drag on Innovative Medicine operational growth was about 920 basis points in the first quarter of 2026 and about 760 in the second1213, and the segment still grew 7.4% and 6.8% operationally in those quarters1415. Each quarter the drag shrinks because there is less Stelara left to lose, while the replacements compound on a larger base. A company that had to find a single blockbuster to replace Stelara would be praying; J&J had Tremfya, the myeloma medicines, Spravato and Caplyta growing at once.
The weakness is concentration of the replacements. Oncology was 42% of Innovative Medicine sales in 20251617, and a single disease, multiple myeloma, accounted for about 86% of oncology's growth between 2023 and 20251819. The machine replaced a psoriasis and bowel-disease drug with, largely, blood-cancer drugs. That works as long as myeloma remains a market in which J&J wins.
The replacement machine is the widest part of the moat, and 2025 proved it under load. The falsifier is the next cliff: Innovative Medicine operational growth in the years Simponi, Opsumit and eventually Darzalex face copies. The segment grew 6.8% operationally in the second quarter of 2026 even with Stelara costing about 760 basis points20; a year of decline through the next expiry would say the machine has slowed.
Stelara's loss was absorbed in 2025; Simponi, Opsumit and Darzalex are the next tests.
