J&J sells 36% of its revenue as devices, but earns about 84% of its segment profit from medicines.

Johnson & Johnson reports two segments, and they are not equal partners. The company is organized into two business segments: Innovative Medicine and MedTech1. In 2025 Innovative Medicine sold $60,401 million and MedTech $33,792 million2, a split of about 64% and 36%3. Profit is split very differently: Innovative Medicine earned $22,266 million before tax and MedTech $4,113 million4, about 84% and 16%5.

Medicines earn five times what devices earn.

The two-segment structure is new. Until 2022 J&J reported three segments, Consumer, Pharmaceutical and Medical Devices6. The consumer business became Kenvue, which J&J floated in May 2023 and exchanged out of in August 20237. The remaining segments were renamed and 2021 and 2022 restated to continuing operations8. The segment chart therefore starts in 2021, and J&J's reported revenue before 2023 includes a consumer business of about $15 billion a year that is no longer part of the company9.

The segments tell different stories about the moat. Innovative Medicine sells patented medicines whose prices are set against payers and governments and whose sales end when the patents do; its margin was 36.9% in 202510. MedTech sells devices into hospitals under negotiated contracts and tenders; its margin was 12.2%11. One is a business of high returns and fixed lifespans, the other of low returns and long ones.

The growth rates have converged recently. In the second quarter of 2026 Innovative Medicine grew 7.8% and MedTech 4.5%12. Over ten years the medicines business grew about twice as fast as devices, roughly 6.8% a year against 3.0% on the old segment bases1314.

The rest of J&J's revenue lines do not exist. Everything the company sells falls into one of the two segments, and segment sales add up exactly to reported sales15. There is no corporate revenue band to explain.

The pages that follow describe each segment. The Moat pages cover the individual franchises in more depth: the replacement of Stelara, the myeloma medicines, the acquisitions, and the device franchises.

The segments also differ in where they sell. In the second quarter of 2026 Innovative Medicine sold $9,979 million in the United States and $6,405 million internationally, while MedTech was nearly balanced at $4,554 million and $4,372 million16. The medicines business depends more heavily on American prices; the device business is more exposed to foreign tenders and currencies.

The quarterly growth rates show the same gap. In the first half of 2026 Innovative Medicine grew 9.4% and MedTech 6.0%17, but segment income rose about 7.5% in medicines and fell about 8.0% in devices1819. The device business is growing its sales and shrinking its profit at the same time.

The mix has shifted a long way in a decade. In 2015 J&J sold $31,430 million of pharmaceuticals, $25,137 million of medical devices and $13,507 million of consumer products20, so medicines were about 45% of the company21. By 2025 they were 64%2223. The consumer business was given away and the device business grew at about 3% a year, so the company that remains is far more dependent on patents and on the prices payers allow for them.

Read together, the two segments say J&J is a medicines company with a device business attached, and the attachment is getting smaller as orthopaedics is separated. The number that would change that description is MedTech's share of segment income, about 16% in 20252425: a rise above 25% after the separation would mean the device business had become a genuine second engine.
