Vision: Contact Lenses Sold Like Consumer GoodsNarrow moat

Johnson & Johnson (JNJ) — moat facet

J&J's contact lens business is a daily habit sold under one brand, and it is putting $1 billion into a Florida factory to keep it.

Vision is the part of J&J's device business that behaves most like the consumer company J&J used to own. It sold $5,468 million in 2025, up 6.3%: contact lenses and other products $3,910 million and surgical products, mainly cataract lenses, $1,558 million1. Contact lens growth was driven by market growth, the Acuvue Oasys 1-Day family and strategic price actions2.

Surgical products grew faster from a smaller base.

The moat is a brand bought repeatedly by the same wearers, prescribed by eye care professionals and replaced every day or every few weeks. A wearer who is comfortable in one lens rarely changes it. That is a switching cost measured in comfort rather than contracts.

J&J is investing to protect it. In the second quarter of 2026 the company announced more than $1 billion to strengthen United States vision manufacturing in Jacksonville, Florida3. The surgical side grew 10.2% in 2025 on recent product innovations4.

The quarter's detail is mixed. In the second quarter of 2026 vision grew 6.0%, contact lenses 6.1%, and surgical products 5.6%; but United States surgical products fell 6.0% while international surgical grew 10.9%5.

The cataract side has new products. In the first quarter of 2026 J&J announced approval of the Tecnis PureSee intraocular lens for United States cataract patients6. Surgical vision grew 10.2% in 20257 but fell 6.0% in the United States in the second quarter of 20268, so the new lens arrives at a useful moment.

Geography helps here. International vision sales were $887 million in the second quarter of 2026 against $565 million in the United States9, and grew 9.1% against 1.4%10. Vision is the one J&J franchise where the growth now comes mainly from outside the United States.

Vision earns steady growth and some pricing from a narrow moat. The risk to that view is contact lens growth without the price actions: the filing lists them as a driver, and if unit growth alone falls below the market's pace, the brand is being harvested rather than grown.

Moat trajectory: Holding steady

Vision grew 6.3% in 2025 and 6.0% in the latest quarter.

Vision franchise sales growth, latest quarter
+6.0%, $1,451M (Q2 2026)

Steady growth; the contact lens business growing without price actions would confirm the brand.
