Johnson & Johnson (JNJ) — threat to the moat
J&J's device business earns about 5 cents a year on each dollar of assets; its medicines earn about 28.
The simplest measure of MedTech's moat is what it earns. In 2025 the segment made $4,113 million before tax on $33,792 million of sales, 12.2%, against 36.9% for Innovative Medicine1. In 2023 MedTech earned 15.4%2.
The assets make it worse. MedTech's identifiable assets were $86,482 million in 2025 against $78,057 million for Innovative Medicine3, so MedTech earned about 4.8 cents of pre-tax income on each dollar of assets and Innovative Medicine about 28.54.
Some of the gap is one-off. MedTech's 2025 income included litigation expense of $0.9 billion, primarily related to the Auris shareholder litigation, and a restructuring-related charge of $0.5 billion5. In the second quarter of 2026 the margin was 13.2%67.
The trend matters more than the level. MedTech's restated pre-tax margin was 15.6% in 2021 and 16.2% in 202289, then 15.4%, 11.7% and 12.2%10. The decline coincides with the Abiomed and Shockwave acquisitions, whose amortisation and integration costs are charged to the segment. J&J bought growth in devices and paid for it partly in margin.
The first half of 2026 went the wrong way. MedTech segment income before tax was $2,416 million against $2,625 million a year earlier, down about 8%1112, on sales up 6.0%13. Selling, marketing and administrative expenses rose to $5,975 million from $5,518 million and research and development to $1,492 million from $1,324 million14.
The margin in the year after the orthopaedics separation will answer the question. Still near 12% would say the problem was never orthopaedics.
