In 2025 J&J paid out $35.9 billion against $19.7 billion of free cash flow and borrowed the rest.

In 2025 J&J spent more than it earned in cash. Free cash flow was $19,698 million: operating cash flow of $24,530 million less $4,832 million of capital spending12. Against that it paid $12,381 million of dividends, $5,953 million of share repurchases and $17,541 million for acquisitions3, about $35.9 billion in total4.

The acquisitions were the borrowed part.

The gap was borrowed. Net debt rose from $12.1 billion to $27.8 billion during the year5, and the company issued approximately $9.2 billion of senior notes to fund the Intra-Cellular deal6.

One year of borrowing is not a problem for a company of this size. The concern is the habit: buying growth, raising the dividend and repurchasing shares all at once is only possible while the balance sheet has room.

The first half of 2026 looked more comfortable. Free cash flow was estimated at about $8.7 billion against $6.2 billion a year earlier7, and net debt was lower than a year earlier at $28.2 billion against $31.9 billion8. But the second half brings the Firefly purchase for $1 billion and $785 million of initial payments to Sail910, and the talc proposal calls for up to $3 billion in 202711.

The pattern is not new in kind, only in size. In 2024 J&J spent $15,146 million on acquisitions and $11,823 million on dividends against free cash flow of about $19.8 billion1213, also more than it generated. Two consecutive years of spending above free cash flow is a strategy, not an accident.

The number to watch is net debt against the $27.8 billion year-end figure; it was $28.2 billion at the end of June 202614. A figure above $35 billion by the end of 2027 would say the purchases are running ahead of the business that pays for them.
