Private benefit managers took $22.3 billion of AbbVie's discounts in 2025, and they also decide which of its drugs patients can get.

The biggest single slice of AbbVie's rebates goes to private payers. Managed care provisions were $22,307 million in 20251, larger than those for Medicaid and Medicare at $21,283 million2. The benefit managers who negotiate them also decide which drugs their plans cover.

Three groups take the discounts.

That gives them leverage at exactly the moments that matter. Humira's access fell as plans moved to exclusive biosimilar contracts3. For Skyrizi and Rinvoq, the benefit managers set the price of preferred placement.

The concentration of this negotiating power is high, and AbbVie's customer for rebates is not the patient or the doctor.

The benefit managers' leverage is also AbbVie's defence against biosimilars. Rebates bought Humira preferred placement long after biosimilars were available abroad, and the Sheet Metal Workers' lawsuit argues that this impaired competition4. If rebates for exclusivity are restricted, AbbVie loses a tool it will want for Skyrizi.

Managed care rebates as a share of net revenue are the thing to track: about 36% in 20255. A rise would mean the benefit managers are winning more of the value of AbbVie's drugs.
