Benefit managers and government programmes took $43.6 billion of rebates from AbbVie in 2025, equal to about 71% of its revenue.
The customers that decide AbbVie's revenue are not the ones that buy its drugs. Benefit managers and government programmes negotiate rebates in exchange for coverage: in 2025 AbbVie's provisions were $22,307 million for managed care and $21,283 million for Medicaid and Medicare1, $43.6 billion together2, about 71% of net revenue3.
Their power comes from the formulary. A plan that excludes a drug removes it for every patient the plan covers, as Humira learned when plans moved to exclusive biosimilar contracts4.
AbbVie's defence is to have drugs that payers cannot exclude. Skyrizi and Rinvoq grew about 24% in the second quarter of 20265 because they are effective treatments that payers have chosen to cover.
The government is also becoming a price-setter directly, through Medicare's selection of Imbruvica, Vraylar, Linzess and Botox6.
The payers also shape which of AbbVie's drugs grow. A drug placed first on a formulary gains patients quickly; one placed behind a rival or requiring prior authorisation grows slowly. Skyrizi's and Rinvoq's 24% growth in the second quarter of 20267 says payers are placing them well, for now.
The public payers behave differently. Medicaid rebates are set largely by statute and rise with list prices; Medicare Part D's redesign and the new government-set prices shift more of the cost to manufacturers. The $21,283 million of Medicaid and Medicare provisions in 20258 is the part of AbbVie's discounts it cannot negotiate.
These payers are AbbVie's most powerful customers. Their share of net revenue, about 71% in 2025, is the figure to watch; a rise would mean more of each prescription's value goes to the payer.
Rebates keep growing faster than revenue.
