Other toxins already compete with Botox in cosmetics, and the medical market is the one they have not yet cracked.

Botox is not the only neurotoxin. AbbVie's 10-K notes that a number of other companies have successfully developed and market products that are being positioned as competitors to Botox1. In cosmetic use they have already taken share; in therapeutic use the clinical evidence and physician habits have protected Botox so far.

AbbVie is also developing a new toxin of its own. Its second-quarter 2026 release lists approvals for trenibotulinumtoxinE, branded Boey, in the European Union and Canada2.

A therapeutic competitor with comparable evidence could compete on price, especially once Medicare sets Botox's price in 20283.

The therapeutic market is harder to enter. A rival toxin must run its own trials in each medical use, and neurologists and urologists are slow to change a treatment that works. That is why the competitors' gains have come in cosmetics first. AbbVie's own new toxin, Boey, shows it expects the category to widen4.

Botox Therapeutic's growth against the neuroscience segment's is the tell: slowing well below the segment would say rivals are gaining.
