Rival toxins have already taken share from Botox Cosmetic in America, where patients pay for themselves.
Botox has competitors, and AbbVie's filing says so: a number of other companies have successfully developed and market products that are being positioned as competitors to Botox1. They compete mainly in cosmetic use, where patients pay out of pocket and clinics choose on price and margin.
The effect is visible in AbbVie's numbers. American Botox Cosmetic sales fell 11% in 2025, partly because of lower market share2. Botox Cosmetic sold $2,602 million worldwide in 20253, while Botox Therapeutic, where the rivals have made less progress, sold $3,769 million4.
AbbVie is responding with its own new toxin. Its second-quarter 2026 release lists approvals of trenibotulinumtoxinE, branded Boey, in the European Union and Canada5.
This is the most price-sensitive part of AbbVie's portfolio, and the competitors have shown they can take share.
AbbVie's defence in cosmetics has been scale and loyalty programmes, and the 2025 decline shows the limits. The 10-K attributes it first to unfavourable pricing from changes to AbbVie's own customer loyalty programme6. Rivals that price below Botox give clinics a reason to switch, and some clinics did.
Botox's reputation is the asset the rivals cannot copy quickly. Clinics that advertise the brand, and patients who ask for it by name, make Botox Cosmetic less exposed than an unbranded product would be. The 2025 share loss shows that the advantage has limits; it does not yet show that it has gone.
Botox Cosmetic's American growth is the measure. A second year of decline would mean the rivals' gains are structural.
