AbbVie's annual report names drug classes rather than companies as competitors. Its immunology products compete with IL-23 inhibitors, IL-17 inhibitors, JAK inhibitors and biosimilars; its oncology products with BTK inhibitors, antibody-drug conjugates and cell therapies1; and a number of other companies market products positioned as competitors to Botox2. Four kinds of rival.

The rivals that matter are four different kinds of relationship. Johnson & Johnson is both a partner, sharing Imbruvica's international profits4, and the main rival to Skyrizi through Tremfya5. The generic makers challenging AbbVie's migraine patents are rivals that wait for the law to let them in6. The other toxin makers compete with Botox where patients pay themselves. And in obesity, AbbVie is the challenger entering a market others already lead, with an early-stage amylin programme licensed for $350 million upfront7.

Competition in this industry is decided mostly by patents and payers rather than by price wars between originators. The Skyrizi versus Tremfya contest is the exception, and it is being fought on clinical data and on the benefit managers' formularies.

The contest also runs through the benefit managers. AbbVie's rebates to managed care were $22,307 million in 20258, much of it paid to secure preferred placement against competing drugs. When a rival offers a larger rebate, the formulary can change without any doctor preferring the rival's drug. That is why competition in this industry often looks like negotiation rather than rivalry.

The moat's competitive test is the immunology growth rate. Immunology grew 15.1% to $8,786 million in the second quarter of 20269; growth falling below the market's while Tremfya accelerates would say AbbVie is losing the next generation of patients.
