UnitedHealth Group (UNH) — moat facet
UnitedHealthcare is 77% of the group's revenue, and in 2026 it is deliberately smaller in order to be profitable again.
UnitedHealthcare is the insurer, and it is most of the company's reported revenue. It had revenue of $344,903 million in 20251, about 77% of UnitedHealth's $447,567 million2. None of that revenue comes from inside the group; it is all paid by governments, employers and individuals.
What is inside it is three businesses. Employer & Individual, the commercial plans, had revenue of $79,228 million in 2025; Medicare & Retirement $171,285 million; and Community & State, the Medicaid plans, $94,390 million3. The company does not report profit by business line, only for UnitedHealthcare as a whole4.
The history is steady growth. Revenue was $222,899 million in 20215 and $344,903 million in 20256, a compound rate of about 11.5% a year7. Most of the growth came from Medicare: Medicare & Retirement grew 23% in 2025 alone8.
How it is paid: mostly premiums. Risk-based premiums are nearly 80% of consolidated revenue9, paid by the government for Medicare and Medicaid members and by employers and individuals for commercial plans; self-insured employers pay fees for administration instead. Premium revenue from CMS was 44% of the company's revenue in 202510.
Profitability is thin and was thinner in 2025. UnitedHealthcare's operating margin was about 5.4% in 2021 and 5.8% in 202211, 5.8% in 2023, 5.2% in 2024 and 2.7% in 202512. In the second quarter of 2026 it earned $3,942 million, a 4.6% margin13.
Growth has now stopped by design. Revenue was $86,017 million in the second quarter of 2026 against $86,103 million a year earlier14, and the January outlook put UnitedHealthcare revenue above $335.0 billion for 202615, below 2025. The company guides to more than $12,000 million of operating earnings16.
The insurer's growth has also been shaped by what it left. Employer & Individual Global, the remaining South American business, had revenue of $9,307 million in 2023 and $3,288 million in 202517 after the Brazil operations were sold in February 202418, and the rest is held for sale19. Domestic Employer & Individual revenue, by contrast, rose from $67,187 million to $75,940 million over the same two years20. UnitedHealthcare's segment assets were $124,051 million at the end of 202521.
The insurer's 2026 profit depends on the medical care ratio more than on revenue. Management guides to a group ratio of 88.1% plus or minus 25 basis points22, against 89.1% in 202523. At 2025's premium level, each point is worth about $3.5 billion24, which is why the guided improvement matters more than any change in membership.
The outlook depends on price, not volume: fewer members at better margins. The verdict would change if the margin failed to recover above 4% while revenue fell, because UnitedHealthcare would then be smaller and no more profitable.
