UnitedHealth Group (UNH) — moat facet
Optum earns about $103 billion a year from outside the group, much of it from insurers that compete with its own parent.
Some of UnitedHealthcare's competitors are Optum's customers. The 10-K says the company's affiliated physician organizations "contract with competitors of UnitedHealthcare"1, and Optum Health serves more than 100 health payer partners2.
The outside business is large. Revenue Optum earned from customers outside the group was about $64.7 billion in 2021, $74.4 billion in 2022, $90.3 billion in 2023, $102.1 billion in 2024 and $102.7 billion in 20253. In 2025 that included about $57.9 billion at Optum Rx, $38.3 billion at Optum Health and $6.5 billion at Optum Insight4.
This relationship is delicate. A rival insurer that sends patients to Optum clinics or runs claims through Optum technology is funding its competitor's parent. The 10-K acknowledges the risk that its physician organizations may fail to maintain relationships with, or adequately price their contracts with, these third-party payer competitors5.
The growth has stalled: outside revenue rose only about 0.6% in 20256, after growing every year before.
The Optum Rx part of the outside business is the largest. Its revenue from outside customers was about $57.9 billion in 20257, from health plans, employers and other clients outside the group. Optum Health's external premium revenue, from other payers taking risk with its clinics, was $4,343 million in the second quarter of 20268.
This is the test of whether Optum has a moat of its own. External revenue is the number to watch: renewed growth would say rivals trust Optum despite its owner; a decline would say they are taking their business elsewhere.
