Optum Health is guided to a 2% margin in 2026 and earned about 4.9% in the first half, on the road back to the 6% it once had.

The second-largest repair job is Optum Health. After losing $1,115 million in 2025 on the 2026 segment basis1, it is guided to more than $2,275 million of operating earnings in 20262. The chief financial officer said in September 2026 that Optum Health margins would be around 2% this year, rising to about 4% in 2027 and 6% the year after3.

Ahead of its own path.

The first half is already ahead of that path. Optum Health earned $2,331 million in the first half of 2026 on revenue of $47,581 million4, a margin of about 4.9%5, and more than its full-year guidance.

The route is shrinking unprofitable contracts, repricing the rest and bringing in partners such as TPG in Florida6. Optum Health earned margins of about 6.7% in 2024 on the 2026 basis7, so the 2028 target is a return to where it was, not a new level.

This bet matters because Optum Health is the part of the integrated model that failed. If it cannot earn a decent margin, the case for owning the doctors weakens.

The guidance for the rest of Optum shows how much of the plan depends on this one segment. The company expects more than $4,925 million from Optum Insight and more than $6,250 million from Optum Rx in 2026, and more than $13,450 million from Optum as a whole8. Optum Health's more than $2,275 million9 is the smallest piece and the one with the most room to move.

The margin path is the measure. A 2026 full-year margin near the first half's 4.9% would put the 2028 target within reach early; a second half that falls back toward 2% would say the first half was flattered.
