UnitedHealth Group (UNH) — threat to the moat
UnitedHealth employs about 165,000 clinicians and still warns of heightened competition for every physician practice.
The clinics are only as good as the doctors in them, and UnitedHealth does not have them to itself. Its 10-K warns of "heightened competition to acquire or manage physician practices" and says its revenues could suffer if it cannot keep satisfactory relationships with physicians or retain members after doctors leave1.
Optum employs a large clinical workforce: nearly 165,000 of UnitedHealth's more than 390,000 employees at the end of 2025 were clinical professionals2. That scale does not stop individual physicians or groups from moving.
The same filing notes that provider organizations may compete directly with UnitedHealth3.
The 10-K also notes that UnitedHealth's affiliated physician organizations contract with competitors of UnitedHealthcare4, so the doctors Optum employs serve other insurers' patients too. That spreads their revenue, but it also means those doctors' loyalty is to their practice, not to UnitedHealthcare.
Optum Health's count of people served will show it first. A continuing decline beyond the planned shrinking would say it is losing doctors as well as contracts.
