Optum Health is where UnitedHealth's 2025 went wrong. On the 2026 segment basis, its operating earnings went from $6,902 million in 2024 to a loss of $1,115 million in 20251. By quarter the decline was steady and then sudden: $1,411 million in the first quarter, $429 million in the second, $41 million in the third and a loss of $2,996 million in the fourth2. The margin went from 5.7% to minus 12.0% within the year3.

A year that ended in a $3 billion quarterly loss.

The business delivers care, mainly to Medicare Advantage patients, often under value-based contracts in which Optum Health is paid a fixed amount per patient and bears the cost of their care. When medical costs rose faster than those payments, the clinics lost money on each patient. A news report attributed the negative margins to rising health costs and federal policies that restricted payments4.

The fourth-quarter loss included charges. The company booked $2,521 million of restructuring and other costs across the group in that quarter5.

The recovery has been faster than the fall. Optum Health earned $1,141 million in the first quarter of 20266 and $1,190 million in the second, a 5.1% margin against 1.7%7.

The number of people served fell as the losses mounted. On the recast basis Optum Health served 95 million people in the first and second quarters of 2025, 93 million in the third and 92 million in the fourth8. Revenue on that basis was $103,516 million in 2024 and $100,051 million in 20259.

The collapse shows the risk in owning the doctors. The margin is the figure to follow: the chief financial officer has said about 2% for 202610; a year running well above that, as the first half has, would say the value-based contracts have been repriced successfully.
