UnitedHealth Group (UNH) — threat to the moat
UnitedHealthcare lost about 1.2 million fully insured commercial members in eighteen months while repricing for higher medical costs.
The commercial members who pay premiums, rather than fees, are leaving. Risk-based commercial membership fell from 8,845,000 at the end of 2024 to 8,165,000 at the end of 2025 and 7,655,000 in June 202612, a loss of about 1.2 million people3.
Some of that is chosen: the company repriced to restore margins after a year in which medical costs ran ahead of premiums4. Some is employers moving to self-insurance, which the fee-based figures capture.
The cost is scale in the part of the book where pricing skill earns the most. Fully insured employers pay premiums that include a margin for the risk the insurer carries; fee-based employers pay only for administration.
The June 2026 quarter showed the decline slowing. Risk-based commercial membership was 7,725,000 in March 2026 and 7,655,000 in June5, a loss of 70,000 in the quarter against about 440,000 in the first three months of the year67. If that pace holds, the repricing is close to finished.
The rate of decline is what to watch. If risk-based commercial membership stabilises near 7.7 million, the repricing is done; if it keeps falling, the business is losing customers it would rather have kept.
