Optum Rx manages $188 billion of drug spending and keeps under five cents of each revenue dollar, most of it from its own parent's members.
Optum Rx is the largest of Optum's segments by revenue and its most mechanical business. It had revenue of $154,726 million in 2025 and operating earnings of $7,193 million, a 4.6% margin, against $116,087 million and $5,115 million in 20231. It fulfilled 1,659 million adjusted scripts in 2025, up from 1,438 million in 202223.
The scale is in the spending it manages rather than the revenue it keeps. Optum Rx managed $188 billion of pharmaceutical spending in 2025, including nearly $87 billion of specialty drugs, through a network of about 64,000 retail pharmacies4. It earns a thin margin on a very large flow.
Most of that flow is internal. About 63% of Optum Rx revenue in 2025 came from the rest of the group5, mainly pharmacy services for UnitedHealthcare's members6. When UnitedHealthcare loses members, Optum Rx loses scripts: quarterly adjusted scripts fell from 414 million in the second quarter of 2025 to 387 million a year later7, about 6.5%8.
The margin has held. Optum Rx earned $1,490 million in the second quarter of 2026, a 3.9% margin against 3.7%9, and the company guides to more than $6,250 million for the year10.
The segment is capital-light compared with the rest of Optum. It spent $399 million on property, equipment and software in 202511, against $1,237 million at Optum Health, and carried $62,262 million of segment assets12. Earning $7,193 million on that asset base13 makes it the highest-return business in the group on this simple measure.
This is a narrow moat built on scale and a captive customer. Script volume is the figure to follow: if it stabilises near 390 million a quarter as UnitedHealthcare's membership settles, the business holds; further declines would say Optum Rx is losing outside clients too.
