UnitedHealthcare passes about half of its revenue to Optum, which makes it the largest customer any Optum business has.

Inside the group the largest customer is the insurer, and the scale of its purchases is striking. Optum's segments recorded $167,956 million of revenue from affiliated customers in 20251, equivalent to about 49% of UnitedHealthcare's $344,903 million of revenue2. Roughly half of every premium dollar the insurer collects is passed to an Optum business for drugs, care or technology.

By segment, affiliated revenue was $96,873 million at Optum Rx, $63,615 million at Optum Health and $12,948 million at Optum Insight in 20253. In the first half of 2026 total affiliated revenue was $77,941 million, against $80,639 million a year earlier4, falling as UnitedHealthcare served fewer members.

As a customer, UnitedHealthcare is guaranteed and enormous, and its prices to Optum are set at management's estimate of fair value5. The internal share of Optum's revenue is covered on its own page; the point here is that Optum's largest client is shrinking deliberately in 2026.

A customer that is also the parent will not leave. It can, however, buy less.

By segment the internal relationship is deepest in pharmacy. Optum Rx's affiliated revenue grew from $55,779 million in 20216 to $96,873 million in 20257, and in the second quarter of 2026 it was $23,242 million against $23,790 million a year earlier8. The drug benefit is where UnitedHealthcare's membership and Optum's revenue move most closely together.

Affiliated revenue is the figure to follow. A stabilisation in the second half of 2026 would say the repricing is complete; continued declines would carry the insurer's shrinking straight into Optum's results.
