Sixty-two cents of every dollar Optum earns comes from its own parent, which is both its advantage and the reason its prices are never tested.

The single most important fact about Optum is who pays it. In 2025 Optum's three segments had combined revenue of $270,620 million, and $167,956 million of it came from other parts of UnitedHealth Group and was eliminated on consolidation1. That is about 62% of Optum's revenue from inside the group2.

Rising toward two-thirds.

The share has risen steadily: about 58% in 2021, 59% in 2022, 60% in 2023 and 2024, and 62% in 20253. By segment it was about 62% at Optum Health, 67% at Optum Insight and 63% at Optum Rx in 20254.

The internal sales are pharmacy care services sold to UnitedHealthcare members by Optum Rx, care delivery sold to UnitedHealthcare by Optum Health, and technology and consulting sold to UnitedHealthcare by Optum Insight, and they are recorded at management's estimate of fair value5.

This is both the moat and its limit. Optum has a guaranteed first customer of enormous size, which lets it build scale no stand-alone rival could reach as quickly. But a business that sells mostly to its parent cannot prove its prices in the open market, and when the parent shrinks, so does Optum: in the second quarter of 2026 the internal share was about 60%6, with affiliated revenue lower as UnitedHealthcare served fewer members7.

The internal flow is visible in the size of the eliminations relative to UnitedHealthcare. The $167,956 million Optum sold inside the group in 20258 was equivalent to about 49% of UnitedHealthcare's revenue9. In January 2026 the company guided to eliminations of about $153,500 million for the year10, smaller because the insurer is serving fewer members.

The second quarter of 2026 showed the dependence working in reverse. Corporate eliminations, Optum's sales to the rest of the group, fell from $41,712 million to $39,648 million11, about 5%12, while UnitedHealthcare's revenue was almost unchanged at $86,017 million against $86,103 million13. The insurer bought less from Optum even though its own revenue held, as it served fewer members at higher premiums.

The internal share is the figure to follow. A falling share with rising external revenue would say Optum is winning outside customers on its merits; a rising share would say it is becoming a cost centre with a separate name.
