UnitedHealth Group (UNH) — threat to the moat
UnitedHealth began 2025 expecting about $29.75 a share and ended with $16.35, which is what happens when an insurer misjudges its costs by a year.
The clearest sign of how badly 2025 went is the path of the company's own forecast. In January 2025 UnitedHealth guided to adjusted earnings of $29.50 to $30.00 a share on revenue of $450 to $455 billion1. In April it cut the range to $26.00 to $26.502. In May it suspended guidance altogether as the chief executive stepped down3. In July it re-established a figure of at least $16.004, raised in October to at least $16.255.
Actual adjusted earnings were $16.35 a share6, about 45% below the original midpoint7. GAAP diluted earnings were $13.238.
A company that prices a year ahead depends on knowing its costs. The forecast failed because medical costs ran above what had been priced: the 10-K says the company observed care patterns above what it expected9.
The market's verdict was swift. UnitedHealth's market value fell from $465.54 billion at the end of 2024 to $299.03 billion at the end of 202510, a decline of about 36% in the year11. The final adjusted result, $16.35 a share, came in slightly above the re-established floor12.
The same thing could happen again. The 2026 guidance, now $19.50 to $20.00 adjusted13, is the test of whether the lesson was learned. A second mid-year cut would say the company still cannot see its own costs.
