Twenty-two million commercial members pay UnitedHealthcare for its network and keep the medical risk themselves, and that number keeps growing.
Much of UnitedHealthcare's commercial business carries no insurance risk at all. Large employers that insure themselves pay UnitedHealthcare to run the network and process claims, and keep the medical cost risk. Commercial fee-based membership was 19,200,000 at the end of 2023, 20,885,000 in 2024 and 21,485,000 in 20251, and 22,265,000 in June 20262.
The risk-based side has gone the other way. Commercial risk-based membership was 8,845,000 at the end of 2024 and 7,655,000 in June 202634, a fall of about 13%5. Total commercial membership barely moved, 29,730,000 to 29,920,00067.
The shift matters for the moat. A fee-based customer rewards the network discounts UnitedHealthcare negotiates, not its skill at pricing risk, and the revenue comes from service fees rather than premiums. The Employer & Individual business had revenue of $79,228 million in 20258, and its domestic part grew from $67,187 million in 2023 to $75,940 million9.
Its strength is breadth: the 10-K lists strong local-market relationships and competitive medical and operating cost positions among the things UnitedHealthcare's position is built on10.
The fee-based business also shows up in services revenue. UnitedHealthcare's services revenue from outside customers was $10,057 million in 202311, and in the second quarter of 2026 it was $2,754 million12. These are fees for running employers' plans rather than premiums for carrying their risk, and they carry no medical cost exposure at all.
This is a narrow advantage that grows quietly. Fee-based membership is the figure to watch: continued growth above 22 million would say employers still value the network most; a decline would say rivals have matched the discounts.
