Optum Rx earns about $58 billion from outside the group and is the most profitable Optum segment, at under five cents on each revenue dollar.
Optum Rx is the pharmacy benefit manager, the largest Optum segment by revenue and the one that earns the most. It had revenue of $154,726 million in 2025, of which $96,873 million came from affiliates1; its revenue from outside customers was about $57.9 billion2, about 13% of the group's revenue3. That external figure is its band in the chart.
What is inside it is pharmacy benefit management, home delivery and specialty pharmacies and infusion services. In 2025 it fulfilled 1,659 million adjusted scripts and managed $188 billion of pharmaceutical spending, including nearly $87 billion of specialty drugs, through about 64,000 retail pharmacies4.
The history is steady. External revenue was about $35.5 billion in 2021, $38.8 billion in 2022, $44.6 billion in 2023, $53.7 billion in 2024 and $57.9 billion in 20255, a compound rate of about 13% a year6. Adjusted scripts grew from 1,438 million in 20227 to 1,659 million in 20258.
How it is paid: by health plans and employers for managing their drug benefits, and on the drugs it dispenses. Products revenue, mainly pharmaceuticals, was $53,380 million for the group in 20259. The company has committed to pass 100% of manufacturer rebates through to clients by January 202810, which will change how part of that payment works.
Profitability is thin and steady. Operating earnings were $4,135 million in 2021 and $4,436 million in 202211, $5,115 million in 2023, $5,836 million in 2024 and $7,193 million in 202512, margins of about 4.5% in 202113 and 4.4% to 4.6% since14.
The outlook is guided at more than $6,250 million of operating earnings for 202615, below 2025. Quarterly scripts fell from 414 million to 387 million between the second quarters of 2025 and 202616, following UnitedHealthcare's membership down. External revenue was about $15.1 billion in the second quarter of 202617.
The quarterly figures show where the 2025 profit came from. Optum Rx earned $1,318 million in the first quarter, $1,441 million in the second, $1,549 million in the third and $2,885 million in the fourth18; the fourth quarter alone was about 40% of the year19. Revenue rose from $35,132 million in the first quarter to $41,456 million in the fourth20. In 2026 it earned $1,192 million in the first quarter21 and $1,490 million in the second22.
The rebate commitment and PBM regulation are the main questions for the line. The 10-K warns that government investigations and studies of PBM practices could result in civil penalties or materially and adversely affect the PBM business model23, and a trade report said the Justice Department was probing Optum Rx billing practices24.
The verdict would change if the margin fell well below 4% after the rebate change takes effect in 2028, because that would show how much of the profit the rebates were.
