UnitedHealth Group (UNH) — moat facet
Optum Health earns about $38 billion from outside the group and twice as much from its own parent, and it lost money in 2025.
Optum Health is the care business, and most of what it earns comes from its own parent. On the old segment basis it had revenue of $101,957 million in 2025, of which $63,615 million came from UnitedHealthcare and other affiliates1. Its revenue from outside customers was about $38.3 billion2, about 9% of the group's revenue3. That external figure is the band in this app's chart.
What is inside it is clinics, including WellMed, home health including Amedisys, and value-based care arrangements in which Optum Health takes a fixed payment per patient and bears the cost of care. Until 2026 it also held Optum Financial and Optum Bank, which moved to Optum Insight on 1 January 20264. It served about 95 million people at the end of 2025 on the old basis5.
The history is rapid growth then a sharp stop. External revenue was about $24.8 billion in 2021, $30.3 billion in 2022, $37.6 billion in 2023 and $41.5 billion in 20246, then fell to $38.3 billion in 20257. Operating earnings were $4,462 million in 2021 and $6,032 million in 20228, $6,560 million in 2023 and $7,770 million in 2024, then a loss of $278 million in 20259.
How it is paid: by insurers, including UnitedHealthcare and more than 100 other payer partners10, under fee-for-service and capitated arrangements, and by patients. Premium revenue from outside customers was $4,343 million in the second quarter of 202611.
Profitability collapsed in 2025 and is recovering. On the 2026 basis the segment earned $6,902 million in 2024 and lost $1,115 million in 202512; in the second quarter of 2026 it earned $1,190 million, a 5.1% margin13.
The outlook is a smaller, more profitable business. Revenue from outside customers was about $9.1 billion in the second quarter of 202614, and the chief financial officer expects margins of about 2% in 2026, 4% in 2027 and 6% in 202815.
It is the most capital-hungry Optum business. At the end of 2025 it carried $100,991 million of segment assets on the old basis16, spent $1,237 million on property, equipment and software, and recorded $1,211 million of depreciation and amortisation17. After Optum Financial moved out, its segment assets at 30 June 2026 were $72,716 million18. Its clinics also treat injured workers, for workers' compensation and no-fault auto insurers and employers19.
The Amedisys acquisition added home health and hospice. It closed on 14 August 2025 for about $3.3 billion, after UnitedHealth agreed to divest 164 locations in 19 states2021. The company is also opening about 15 clinics a year in Florida22, so the segment is shrinking in contracts while still building in chosen markets.
The verdict would change if the margin stalled below 4% in 2027, because the value-based model would then be earning too little to justify owning the doctors.
