Intuitive Surgical (ISRG) — threat to the moat
Intuitive proved robotic surgery works, and that proof is now a public good.
Intuitive spent twenty-seven years teaching the surgical profession that robotic assistance works. That was necessary, it was expensive, and the benefit of it does not belong exclusively to Intuitive.
The company puts the paradox in its own words: "we believe that the entrance or emergence of competition validates MIS and robotic-assisted surgery or robotic-assisted bronchoscopy"1. Validation cuts in the direction of the category, not the incumbent. A surgeon who understands the console paradigm, tremor filtration, motion scaling and wristed instruments has acquired a general competence; the specific interface is the smaller part of it.
The most valuable teaching Intuitive did was of clinical evidence, and that is a public good. Peer-reviewed outcome studies establishing that robotic prostatectomy or robotic hernia repair produces better results are studies a competitor can cite. The 10-K's own framing — that success depends on "educating hospitals, surgeons, and patients on the demonstrated results associated with robotic-assisted medical procedures"2 — describes work that, once done, does not need doing again.
What remains proprietary is the specific interface fluency, the training record, and the relationships. Those are real and they are why displacement takes years rather than quarters.
Grade this on whether Intuitive's installed base keeps growing faster than its rivals' — 11,710 systems at mid-2026, up 12%3 — and, more subtly, whether new programmes still start with a da Vinci. A teaching hospital that begins a robotics programme on somebody else's console is the leading indicator of everything on this page.
