✦ Force Feedback, and the Certification Europe Has Not GivenNarrow moat

Intuitive Surgical (ISRG) — the future bets

The flagship's distinguishing feature is explicitly excluded from its European certification.

Da Vinci 5's distinguishing capability is force feedback: instruments that "enable surgeons to sense and measure the force exerted on tissue during surgery"1. In a system whose whole premise is that the surgeon's hands are somewhere else, restoring the sense of touch is the most significant addition since the wristed instrument.

The flagship's distinguishing feature cannot be sold into the markets performing 36% of the procedures. A certification, not a technology, is the constraint.

The rollout is staged and incomplete. FDA clearance came in March 2024 for all specialties indicated for da Vinci Xi except cardiac and pediatric; South Korea followed in October 2024 and Japan in June 20252. European certification under the EU MDR arrived in July 2025 — for adult and pediatric use across abdominopelvic and thoracoscopic procedures, but "excluding the use of force feedback", with Intuitive stating that it intends to seek certification for it in future.

So the flagship's flagship feature is not yet available in Intuitive's second-largest market, and the international launch is proceeding "in the midst of a phased launch of our da Vinci 5 surgical system over several quarters".

That is a timing constraint rather than a strategic one, and the American evidence suggests it is worth waiting for: da Vinci 5 was 870 of 1,721 placements in 2025 and 246 of 468 in the June 2026 quarter3, and average selling price rose from about $1.50 million to about $1.60 million, which Intuitive attributes partly to da Vinci 5 mix4.

It also feeds the service annuity, since a newer and more capable system carries a larger contract — the company names da Vinci 5 mix as a driver of 2025's 20% service revenue growth5.

Track da Vinci 5 as a share of placements outside the United States. The domestic share is already past half; the international share is the number that says whether the upgrade cycle has a second act.

Moat trajectory: Widening

Force Feedback instruments are shipping in the United States on da Vinci 5, which is now half of placements. The constraint is a European certification that has not arrived rather than a technology that does not work.

Force Feedback ships on da Vinci 5, so this base is how many operating rooms can use it. European certification would open the feature to a large share of the base placed outside America.
