A reader watching systems revenue is watching an accounting choice at least as much as a customer decision.

"Revenue for systems sold or placed under a sales-type lease arrangement is recognized upfront whereas revenue for fixed-payment operating lease arrangements is recognized on a straight-line basis over time. Therefore, in a period when the number of operating lease placements increases as a proportion of total system placements, total systems revenue is reduced, which can create volatility in the systems revenue recognized in any given period."1

That is exactly what happened in reverse in 2025. Systems revenue grew 26% to $2,473.7 million, and the company attributes the increase to "an increase in da Vinci surgical system placements, including a decrease in the proportion of da Vinci surgical system placements under operating leases, higher operating lease revenue, and higher ASPs"2. The second of those four items is a mix effect, not demand.

The June 2026 quarter went the other way: operating-lease placements rose to 54% of the total3, which suppresses the reported line even as unit placements grew 18%.

Add a third distortion: lease buyouts, where a customer exercises an option to purchase a system it has been leasing. Those were $130 million in 2025 against $109 million in 20244, and Intuitive expects them to "fluctuate period to period."

None of this is improper and all of it is disclosed. It means systems revenue should not be read as a demand signal at all. Read placements and the installed base instead.
