The most forward-looking element of Lilly's incretin dominance is that it is racing to obsolete its own drugs before any competitor can, through a deep next-generation pipeline. An oral GLP-1 pill, orforglipron1, could dramatically widen access by replacing injections with a convenient tablet; a triple-hormone drug, retatrutide, has shown even greater weight loss in trials; and a broadening set of indications extends the franchise into new diseases. Lilly is not defending its lead statically — it is trying to extend it by out-innovating the entire field, itself included.
This strategy is the right one and a genuine strength. In pharmaceuticals, the only durable defense of a franchise is to keep inventing its successors, and Lilly's next-generation incretin programs are among the most advanced and ambitious in the industry. An oral version alone could transform the market — pills are cheaper to make, easier to distribute, and far more acceptable to the many patients who resist injections — and could open the obesity opportunity to hundreds of millions more people while extending Lilly's patent-protected lead by years.
The caveats are the ordinary ones of drug development: pipeline drugs can fail in late trials, competitors are pursuing the same next-generation targets, and the very success of a cheaper oral drug could cannibalize the profitable injectables. But the direction is exactly what a great pharmaceutical company should be doing — spending its blockbuster profits to invent the drugs that will replace its blockbusters. Lilly's next-generation pipeline is the clearest evidence that it understands its moat is temporary and is working hardest to renew it, and the strongest reason to believe the incretin franchise can endure beyond the current molecules.
Widening. Lilly is racing to obsolete its own incretins before anyone else can — an oral pill to reach the injection-averse, a triple-hormone drug that beats the current best, a broadening set of indications. These programs are advancing and, if they deliver, extend the franchise and the patent-protected lead by years. Pipeline drugs can fail, and rivals chase the same targets, but Lilly's next-generation incretin bench is among the most advanced in the industry and is actively widening the moat.
Lilly buys as well as builds its next generation. A sustained rise means more of the pipeline is being bought from outside.
